LHM Hospice & Social Services Foundation (LHM Foundation) proudly announces the launch of our Horizons transitional housing program and Expanding Horizons fundraising campaign. The new program will provide individualized services designed to help participants overcome barriers to housing security in a supportive residential community. Through a comprehensive approach that addresses immediate needs and provides the skills and resources to empower long-term stability, the Horizons program will focus on creating bright, sustainable futures for all participants.

“At LHM Foundation, we celebrate the fact that every person has a light within them. Unfortunately, life’s hardships and losses can make it harder to see. LHM Foundation’s Horizons program was created with a vision to walk alongside those whose light has been diminished by the threat of homelessness,” shared Amy Foerster, CEO of LHM Foundation. “There is a transitional housing shortage in our communities, such that many in need go without supportive programming to help them overcome barriers to stable housing. Our Horizons program will provide a vital resource to individuals and families across South Central PA facing homelessness and housing insecurity.”

Participants in the Horizons program will be assigned a success advocate who will help them establish individualized goals and create a path towards achieving them. They will engage in programming on topics tailored to their needs, such as financial literacy and money management, educational assistance, job coaching, and tenant/homeowner rights and responsibilities, while living in an environment that encourages spiritual, emotional and psychological growth.

Foerster shared that Luke Drayer, chair of LHM Foundation’s Board, has contracted to purchase a 42,000 square-foot former senior living facility in Harrisburg featuring 58 resident units for single adults and families, a commercial-grade kitchen and dining room, employment and education readiness spaces, and multiple gathering spaces to foster community. “We are blessed by the generosity of Luke and his wife, Kelly, in donating the use of the facility to LHM Foundation so that Horizons participants can live in a secure and supportive environment throughout the program,” said Foerster.

“For an increasing number of individuals and families in our area, the threat of homelessness is real. I am proud that LHM Foundation is committed to addressing unmet needs in our communities,” said Drayer. “I look forward to witnessing and supporting the meaningful impact the Foundation’s transitional housing program will have on participants.”

In conjunction with the announcement of our transitional housing program, LHM Foundation is launching the Expanding Horizons fundraising campaign. The campaign, which includes a broad range of naming and sponsorship opportunities, seeks to raise $10 million to open the doors of the family care facility by October 2025. A generous donor is matching dollar-for-dollar the first $3 million received through the campaign so that donors can double their impact.
