LHM Hospice

Typically, hospice care is appropriate when a patient’s disease or illness is no longer responsive to curative treatment, the patient has chosen comfort-based care, and the patient’s physician has estimated a life expectancy of six months or less. Too often, misconceptions and lack of information prevent patients and their families from accessing hospice as early as possible, such that they do not receive the full supportive benefits of this specialized care.

Any advanced-stage illness may qualify for hospice care, but advanced- or end-stage cancers, cerebrovascular disease/stroke, renal disease, congestive heart failure, chronic obstructive pulmonary disease (COPD), and dementia are the conditions most seen in hospice.

Hospice care is often provided in the patient’s home but may also be provided in an assisted living facility, hospital, or nursing home.

A hospice nurse will reach out to schedule an initial visit at a time that’s convenient for the patient, family, and caregivers. During the visit, the hospice nurse will perform a comprehensive patient assessment, explain hospice services, discuss the patient’s specific needs, and help to complete admission paperwork.

Every LHM Hospice team member plays an important and unique role in providing care to hospice patients. With the patient and their family, LHM Hospice will develop and implement a hospice plan of care tailored to the patient’s specific needs. The LHM Hospice team members involved in delivering the plan of care include:

• The Hospice Medical Director oversees the hospice plan of care.

• The Hospice Nurse manages all aspects of the patient’s care and collaborates with the patient, family, skilled nursing facility staff (if applicable) and other hospice team members to develop and coordinate the hospice plan of care.

• The Hospice Aide (a certified nursing assistant (CNA)) provides services to the hospice patient as determined by the plan of care, which might include personal hygiene, bathing, dressing, feeding, hair care, oral care and companionship.

• The Social Worker assesses psychosocial needs and assists with long-term planning, funeral arrangements, and other supportive services.

• The Chaplain provides spiritual and emotional support, comfort and solace to patients, families and caregivers.

• Hospice Volunteers play an important role in caring for hospice patients and their loved ones, which might include providing companionship to patients, respite for caregivers, and much more.

• Other professionals, including Physical, Occupational, Speech and Respiratory Therapists and a Dietician, are available as needed.

• Bereavement Counselors provide ongoing support for loved ones following the patient’s passing.

Medicare, Medicaid, and many private insurance plans cover hospice care with few, if any, out-of-pocket expenses.
