Horizons Transitional Housing Program:
A Vital Resource
LHM Foundation’s transitional housing program – Horizons – provides a critical resource for individuals and families facing homelessness in South Central Pennsylvania. The Horizons program offers a supportive residential community within our family care facility, as well as individualized services and supportive programming tailored to help participants overcome their barriers to housing stability.
Through a comprehensive approach that addresses immediate needs and provides the skills and resources to empower long-term stability, the Horizons program helps participants create bright, sustainable futures.
LHM Foundation will expand the opportunities – the HORIZONS – available to each transitional housing program participant with respect to improved well-being, financial stability, and quality of life.
What is Transitional Housing?
Transitional housing programs provide case management and supportive services in a safe residential environment, giving unhoused individuals the opportunity to transition into – and maintain – secure housing. Programs focus on preventing homelessness by addressing its root causes on an individualized basis.
The Horizons Family Care Facility
The home of the Horizons program and LHM Foundation’s administrative offices, our family care facility provides a safe and supportive living environment where Horizons participants can better focus on improving their individual situations, with the goal of transitioning to stable housing upon program completion. The 42,000 sq. ft. facility is situated on a quiet, 15-acre campus and features:
- 58 Resident Units Customized for Single Adults & Families
- Multiple Gathering Spaces to Foster Community
- Employment & Education Resource Space
- Commercial-Grade Kitchen & Dining Room
The Horizons Program: Creating Bright, Sustainable Futures
Through a comprehensive approach that addresses immediate needs and provides the skills and resources to empower long-term stability, Horizons focuses on helping program participants thrive. When individuals or families join the Horizons program, they will work with their assigned case manager to establish their individualized goals and a path toward achieving them. LHM Foundation staff and volunteers will provide guidance as participants are held accountable for making meaningful progress toward their goals and establishing relationships with relevant community resources. LHM Foundation offers supportive programming on topics such as:
- Financial literacy/money management
- Healthy living environments (e.g., parenting, cleaning, cooking, etc.)
- Tenant/homeowner rights and responsibilities
- Job coaching/vocational assistance
- Educational assistance (e.g., GED preparation, coursework tutoring, and school enrollment)
- Spiritual and emotional growth
