LHM Hospice & Social Services Foundation (LHM Foundation) proudly announces that our hospice program has achieved accreditation through the Accreditation Commission for Health Care (ACHC), a nonprofit organization that has stood as a symbol of quality and excellence since 1986. ACHC accreditation is a voluntary process through which hospice providers can demonstrate their dedication to and compliance with rigorous standards of performance and patient care. Among other steps, the accreditation process included a three-day site visit during which an ACHC surveyor carefully reviewed our policies and patient medical charts, conducted staff interviews and attended select patient care visits.
“I am incredibly proud of our hospice team,” said Kris Bouslough, Executive Director of Hospice Services. “We first came together in November 2023, intending to create a different kind of hospice provider, one that manifests our Foundation’s mission to provide spiritual, emotional and physical care to patients and their families throughout central Pennsylvania. With hard work and an unwavering commitment to excellence, we achieved ACHC accreditation six months after seeing our first hospice patient. And we are growing a culture of exceptional hospice care that demonstrates our ongoing dedication to ACHC’s high standards.”
We are a 501(c)(3) nonprofit organization born out of a desire to identify, implement and sustain solutions for unmet needs in our communities. Our hospice program has a specially trained team of clinicians, social workers, chaplains, volunteers and others who provide physical, spiritual and emotional support to patients in the last stages of an incurable condition. Our team’s approach to care is driven by a desire to help patients live as fully as possible, prioritizing their comfort and quality of life each day.
In addition to delivering our inaugural hospice program, we also deploy volunteer teams into the community to assist other charitable organizations deliver on their missions. We are currently developing a third program, which will provide those who are unhoused or at imminent risk of becoming unhoused a supportive framework for setting and achieving goals intended to overcome their barriers to housing security while living in program housing.
